GLP-1 injections like semaglutide and tirzepatide can lead to extra hair shedding for some people, but the medication is not attacking your hair follicles. The shedding is usually a temporary reaction called telogen effluvium, set off by how quickly you lose weight and the nutrition gaps that come with eating much less. It almost always grows back, and losing weight at a steady pace, keeping your protein up, and checking the nutrients your provider follows in your labs can lower the odds it happens to you.

Is the Medication Attacking My Hair, or Is Something Else Going On?
The reassuring part first: GLP-1 medications do not poison or shrink your hair follicles the way some hair-loss conditions do. What most people notice is telogen effluvium, a temporary, even shedding across the whole scalp rather than bald patches or a receding line.
Your hair grows in cycles. At any moment most of it is actively growing, and a smaller share is resting before it naturally falls out. A sudden physical stress can push more follicles than usual into that resting phase at once. A few months later they let go together, which is why the shower drain looks alarming even though nothing is wrong with the follicles themselves.
On a GLP-1, that stress comes from the weight loss itself. The drug is not doing something to your scalp; a fast, large drop simply sends a stronger signal to the hair cycle. That is one more reason the rate of loss matters, and why it helps to know how much weight you can expect to lose and over what kind of timeline. Eating far less also means fewer of the building blocks hair needs, so low protein and thin iron, zinc, or vitamin D stores add to the same effect.

How Common Is Hair Loss on GLP-1 Injections, and How Bad Does It Get?
Most people on GLP-1 injections do not notice significant hair changes at all. In studies, hair loss was reported by a minority of patients, and it showed up more often in people who lost a large share of their body weight quickly than in those who came down gradually.
When it does happen, it usually looks like more hairs than normal coming out when you wash or brush, and your ponytail feeling thinner, rather than visible gaps. It's diffuse, it tracks the pace of change, and for most people it clears up on its own. That's very different from permanent, patterned hair loss, so it's worth keeping the two apart before you panic.
When Does the Shedding Start, and Will My Hair Grow Back?
Telogen effluvium runs on a delay. Because the follicles rest before they release, shedding usually begins a few months after the trigger, often around the three to six month mark once weight loss is well underway, and tends to peak a little after that.
For most people it's temporary. As your weight stabilizes and your nutrition steadies, the hair cycle resets and the follicles move back into their growth phase. Regrowth is gradual and can take several months to a year to fully catch up, since hair grows slowly. If shedding keeps going well past that window, or you see patches or a widening part, that is worth having checked, because something other than the diet shift may be involved.
What You Can Do to Protect Your Hair
You have real levers here, and most of them line up with losing weight in a healthy way in the first place. None of them is a guarantee, but together they lower the odds and the severity.
- Lose weight at a steady pace. Since the shedding tracks with how fast and how much weight comes off, an unhurried rate is protective. Much of this comes down to your dose, so raising the medication gradually rather than rushing to a high one gives your body, and your hair, time to keep up.
- Keep your protein up. Hair is built mostly from protein, and appetite drops sharply on these medications, so protein is the easiest thing to fall short on. Prioritizing it at each meal supports both your hair and the muscle you want to hold onto while you lose fat. Building diet and exercise alongside your injections in from the start makes this far easier to sustain.
- Cover the key nutrients, guided by your labs. Low iron, zinc, vitamin D, and B12 all feed into shedding, and they are common when you are eating much less. Rather than guessing at supplements, it is safer and more useful to have these checked and correct only what is actually low for you, at the right amount.
- Be gentle with your hair. While it is shedding, easy handling helps: loose styles, less heat, and no tight pulling. It will not change the underlying cycle, but it avoids adding stress on top of it.

How a Supervised Program at New Horizons Wellness Center & MediSpa Lowers the Risk
Almost every step above works better when a provider is watching the whole picture, which is the difference between a mail-order prescription and a GLP-1 and dual-agonist weight loss program that is managed in person. At New Horizons Wellness Center & MediSpa, the program starts with labs, including a complete blood count, a metabolic panel, thyroid, A1C, and a lipid panel, plus a complete body composition analysis, so the nutrient and thyroid factors behind shedding are on the radar from day one rather than discovered after your hair thins.
From there, your dose is raised to match how you respond instead of on a fixed rush, and you are seen monthly for follow-up and refills. Body composition is re-checked over time so the goal stays fat loss with your muscle protected, and you get guidance on protein, diet, and resistance training as part of the plan.

Emile Kettle, PA, our Center Director, and Shawn Terry, PA lead the weight-loss and wellness care under the medical oversight of Dr. Jonathan Cheng, MD, our Medical Director. Many of our weight-loss patients have stayed with the practice for years, and some drive from across the Houston metro to keep their visits. Individual results vary, and protecting your hair is part of the same plan that protects your muscle while you lose fat.
"I really like the body machine that is used at increments of 3 months to see fat loss and labs to compare."

Common Questions
Should I stop my GLP-1 if my hair starts shedding?
Usually not on your own. For most people the shedding is temporary and improves as weight stabilizes and nutrition steadies, so stopping is rarely the answer. Bring it to your provider, who can look at your pace of loss and your labs and adjust the plan while keeping your results.
Does everyone lose hair on these medications?
No. Most people do not notice meaningful hair changes. When it happens, it is more common with fast, large weight loss and with low protein or nutrient stores, which is much of what a supervised plan is set up to manage.
Will biotin or hair vitamins fix it?
If your shedding comes from a specific low level, correcting that nutrient helps, which is why testing beats guessing. Biotin on its own has limited evidence for this kind of shedding, and more is not better with several of these nutrients. Ask your provider what your labs actually call for before adding supplements.
Can I prevent hair loss completely?
There is no guarantee, since some of it comes with losing a meaningful amount of weight. What you can do is make it less likely and less severe by pacing the loss, keeping protein up, and correcting any real deficiencies, and in most cases the hair grows back.
